APS currently serves as the contracted provider of Mental Health Case Management/Care Coordination services in Cecil County.
Care Coordination utilize a wraparound model of care to assist families with developing a team of community and professional supports to best meet the needs of the child.
The care coordinator will lead regularly scheduled Child and Family Team meetings designed to assess progress, make recommendations, access resources, and implement a plan to address concerns or areas of need.
Some children will also qualify for enhanced services including enhanced therapies and in-home supports.
