Adult PRP services are available to those individuals who are 18 years of age and older, who are receiving continuous care for mental health services from a lincensed physician or licensed clinician, who have or are eligible for medical assistance, and who meet criteria for a priority population mental health diagnosis. Services are offered in multiple settings/combination of settings in order to best meet the needs of the client (i.e. onsite day program, offsite outings, in-home, etc.). Following a period of assessment which evaluates client strengths and preferences, individualized rehabilitation goals are implemented.
Target areas may include but are not limited to the following:
Development of natural supports through increased opportunities for socialization
Connection to community resources
Navigation of the entitlement system
(i.e. SSA, DSS, local housing agencies)
Case management
AdvocacyLife skills training (i.e. grooming, hygiene, nutrition, dietary
planning, budgeting/money management, self-administration of medication)
Development of crisis management and wellness self-management/recovery related skills Assistance with scheduling and maintaining appointments
Coordination of care with other providers/treatment team members (i.e. therapist, psychiatrists, PCP, substance abuse providers, specialists)
Mobility skills training (i.e. assistance with applying for various forms of reduced cost transportation, assistance with understanding the bus system, etc.)
Vocicational Skills
